Appointments System

Patients will be triaged by a Healthcare professional on the same day MON - FRI

To make an appointment for morning surgery please telephone after 08:00.

Telephone Advice

The doctors will give you advice on the telephone. Please telephone the surgery on 01782 917840.  The doctor will call you back as soon as they are able to but will give priority to urgent calls.  If it is more appropriate for the doctor to call you back in the afternoon please inform the staff when requesting a call back.  Please make sure that we have a correct contact number for you.

Urgent Appointment

To request an urgent appointment for today or tomorrow (Monday to Friday):

  • use our appointment request form, Monday to Friday from 8am to 6pm. We will respond within 4 hours during opening times
  • phone us on 01782 917840, Monday to Friday from 8am to 6pm
  • visit the surgery and speak with a receptionist, Monday to Friday from 8am to 6pm

When you get in touch, we’ll ask what you need help with.

We will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.

Routine Appointment

To request a routine appointment in the next 7 days:

  • use our appointment request form, Monday to Friday 8am to 6pm. We will respond within 4 hours during opening times
  • phone us on 01782 917840, Monday to Friday from 8am to 6pm
  • visit the surgery and speak with a receptionist, Monday to Friday from 8am to 6pm
  • use your NHS account (through the NHS website or NHS App) to book a screening test or vaccination

When you get in touch, we’ll ask what you need help with.

We will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.

Home Visits

If possible please try to telephone reception before 10am if you require a home visit.

A doctor or nurse may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.

House visits are only available for patients who are housebound because of illness or disability.

Cancel an Appointment

It is important that you inform the reception staff if you are unable to attend your appointment, this will allow that appointment to be offered to another patient. If you fail to notify the Practice that you are unable to attend, you will be sent a letter informing you that you have defaulted from your appointment. Persistent defaulters maybe removed from the list.

You can also complete our appointment cancellation notification request form. This can only be used if your appointment has been arranged for more than 24 hours in advance. (excluding weekends and public holidays). You can also text to cancel your appointment.

Late For Your Appointment

Please attend your appointment on time, if you are late you may not be seen. If you are not seen you will not be able to rearrange your appointment until the next working day-except in the event of an medical emergency that requires immediate attention.

Interpreter

If you require an interpreter to attend with you when you see your Doctor please notify the receptionist and she will arrange this for you.

Extended Access

 

What is Extended Access?

Extended access is the offer, to registered patients of a practice, of pre-bookable appointments outside of core contractual hours, either in the afternoon or evening and at weekends

There are 2 locations of access ‘Hubs’ throughout Staffordshire:

  • Haywood Hospital
  • Bradwell Hospital
